First Visit

Please help us during your initial visit by providing the following:

• The referral form from your dentist.
• All the existing radiographs. If your dentist has your radiographs in digital form, it would be helpful if they could be sent via e-mail to our clinic.
• A list with all the medications you are taking. Inform us about any health issues that need special treatment (e.g. artificial heart valves, heart disease, if you need to be under antibiotic prophylaxis, if you suffer from diabetes or hypertension, etc.).
• Documents you would like us to fill.

Please all patients under 18 years of age must be accompanied by a parent or guardian.
